Contacting us through the form

When you submit the "Talk to the Team" form, you ask us to contact you about the Salvation program. Submitting the form does not enroll you in anything, and it does not obligate you or us. You agree that the information you give is accurate and that it is yours to give. Please do not include health information in the form. Text messages and email

If you agree to receive texts or email, you can stop at any time. Reply STOP to a text, or use the unsubscribe link in an email. Message frequency varies. Message and data rates may apply. Consent is not a condition of purchase.

Availability

Medical services connected to the program are currently available only to residents of Georgia and South Carolina, and only through an independent licensed physician. Information on this site about services is not an offer to provide services anywhere they are not available.

The text, images, logos and design on this site belong to Golf Partners Inc. or its licensors. You may view the site for personal use. You may not copy, republish or use our content for other purposes without written permission.

Acceptable use

Do not misuse the site, attempt to disrupt it or gain unauthorized access, submit false or someone else's information, or use it to send spam.
